Vancomycin is an antibiotic used primarily For staphylococcal infections. It also is recommended by the American Heart Association For the prevention of subacute bacterial endocarditis in susceptible patients who are allergic to penicillin. Anaphylactic reactions to vancomycin are rare, with only two cases reported in the literature. A case of an anaphylactic reaction to vancomycin in a chiht patient during general anesthesia For dental surgery is reported. The patient had a ventricular septal defect and a penicillin allergy. Vancomycin IV was administered For prophylactic coverage while the patient was under general anesthesia. Anaphylactoid Shock with Infusion of 5% Albumin in a Patient under General Anesthesia

A 61 years old male patient undergoing gastrectomy under general anesthesia developed severe anaphylactoid shock after 15 minutes from starting 5% albumin infusion. His blood pressure went from 101/67 down to 44/23 with his heart rate going up from 91 to 99. A total of 1600 mcg of phenylephrine were given without any improvement in the hemodynamics. We then gave 50 mcg of Epinephrine to which b...

General anesthesia with remifentanil for cesarean section in a parturient with severe mitral and tricuspid regurgitation

Valvular heart lesions have deleterious effects on hemodynamics in parturients during pregnancy. Cesarean section with an opioid-based general anesthesia is used to alleviate the adverse effects. We described a case of a 28-year-old primigravida at 37 weeks’ gestation with a diagnosis of severe mitral regurgitation, tricuspid regurgitation, and pulmonary hypertension in active labor. Cesarean s...
